Third Symptomatic congenital heart block (CHB) AV block of any degree that is expected to resolve and unlikely to recur (e.g., drug toxicity, Lyme disease, sleep apnea) without treatment

Acquired asymptomatic CHB
Neuromuscular diseases with AV block, with or without symptoms
